2nd: You can either call us to schedule your appointment or you can stop by our office to schedule. If you are calling to schedule you can bring in your filled out " New patient Intake Form" and the " HealthQuest Questionaire" with you on your first visit. If you are planning on stopping in at our office to schedule your appointment, please bring your filled out " New Patient Intake Form" and the " HealthQuest Questionaire"  with you when you come to schedule.
