1st: Print out either a male or female version form and a HealthQuest Questionaire, and fill it out at home.

2nd: You can either call us or stop by our office to schedule your appointment. If you are calling to schedule, you can bring in your filled out " New patient Intake Form" and the " HealthQuest Questionaire" with you on your first visit. If you are planning on stopping in our office to schedule your appointment, please bring your filled out " New Patient Intake Form" and the " HealthQuest Questionaire"  with you when you come to schedule.



* Note: If you don't have a printer and are unable to print out the forms then just stop in to see us and we will be happy to print it for you.
